NETWORK DESCRIPTION

The Waterloo Wellington Dufferin Hospice Palliative Care Network is a partnership of community palliative care stakeholders both individuals and organizations that are committed to providing a full range of quality palliative care services to patients/clients/residents and their families. It is a collaborative, innovative and patient/client/resident focused approach to improving the palliative care system in Waterloo Region and Wellington Dufferin Counties. The Network creates an operating plan and priority list of initiatives that will be implemented across the community. The community partners have worked together to develop a conceptual model based on the Square of Care of Organization from the Canadian Hospice Palliative Care Association (CHPCA); A Model to Guide Hospice Palliative Care (March 2002).
