End-of-life Care Symptom Response Kit (SRK)
Symptom Response Kits have been developed to assist in-home clients who could experience unexpected end-of-life symptoms or rapidly escalating symptoms. The kit was developed for patients in the Waterloo Wellington Dufferin area received input from Waterloo Wellington CCAC, Nursing Contract Agencies, Pain and Symptom Management Consultants of Waterloo Region and Wellington-Dufferin, Pharmacists, and Palliative Care Physicians.
The kit is a small tool box which contains end-of-life care medications and some medical supplies. The kit is made available to patients by it being pre-ordered by their physician. The medications available in the kit allow for community nurses to respond quickly to unexpected symptoms or change in patient’s condition and may avoid visits to the Emergency Department.
The kits were implemented in April 2006. The order forms for physicians can be found and downloaded from www.hpcconnection.ca .
