Vision

The Waterloo Region-Wellington-Dufferin Hospice Palliative Care Network will provide leadership and structure to facilitate the ongoing development and implementation of a comprehensive, integrated and coordinated system of hospice palliative care / end-of-life care for the community.

1. Facilitate the coordination and integration of hospice palliative care/ end-of-life care services at a systems level
2. Assess and monitor the community’s needs
3. Promote service innovations
4. Provide a forum for receiving and responding to Ministry of Health and Long-Term Care policy initiatives and for participating in the review and development of documents related to palliative care
5. Provide a forum that builds on existing relationships and facilitates ongoing collaboration and information sharing between stakeholders and across care settings
6. Develop process mechanisms for resolution of issues across disciplines and organizational boundaries related to the development and implementation of standards, guidelines and related initiatives in the provincial strategy
7. Support and co-ordinate educational opportunities for consumers, families, caregivers, health care providers and volunteers
8. Create and participate in regional opportunities for consultation on Hospice Palliative Care issues such as policy development
9. Identify potential priority areas and issue specific projects
10. Establish financial sustainability
